When a parent refuses help, the agency you pick has to handle the refusal as well as the job. Hourly rates are easy to compare and often close to each other. The differences that matter sit in the terms: screening, supervision, replacement, and exit. Put all the agencies on the same scorecard so you are comparing answers, not sales pitches.

Local option: New Plan Care gives families in the St. Louis area in-home help so a parent can stay home: companionship, help with bathing and dressing, meals, and overnight care. Call (314) 405-0887.

Screening and registration

Ask what background checks the agency runs on each caregiver, who pays for them, how often they are repeated, and whether the results are shared with you. Ask what license or registration, if any, applies to the agency in Missouri and get the number in writing so you can check it yourself. Ask whether the agency carries liability and bonding insurance and request the certificate.

Supervision and who you call

Ask how often a supervisor visits the home, what the visit includes, and whether the caregiver's notes come to you. Ask who answers the phone when the caregiver is out sick and what the response time is. Ask for the name of the person you would call on a Saturday, not only the office line.

What the caregiver will and will not do

Ask for a written list of tasks. A non-medical caregiver may give medication reminders and pick up prescriptions as an errand. Medicine itself stays with Dad, the family, his doctor, and his pharmacist. Ask the agency to confirm that split in writing, and check that the list does not blur it.

Replacement and refusal

Ask what happens when Dad refuses to open the door or sends the caregiver away. Ask how fast the agency sends a replacement, whether you pay for the missed visit, and whether the agency reports the refusal to you the same day. This is the test most agencies skip in a sales call, so ask it directly and write down the answer.

Exit terms and final bill

Ask for the notice period, any deposit, how the final invoice is calculated, and whether you can end service without penalty if Dad's needs change. Read whether the contract renews automatically and how you cancel it. Keep a copy of every answer with the date and the name of the person who gave it.

Your next move

Write the eight questions above on one page, call three agencies, and ask each the same questions in the same order. Score each answer as clear, vague, or missing, and drop any agency that will not put its answer in writing. Use the Local Help in St. Louis page to find agencies serving your part of the metro, then book the one with the most clear answers for a short trial.
